Please report all practice or game related injuries to your Coach and the Safety Director within one week of the incident if the injury required professional medical attention.  The player's coach is responsible for completing the Incident Report Form (link below) and submitting to the Safety Director and Regional Commissioner.

Any player returning to AYSO play from an injury that required physician care or visit to a medical facility requires a Participation Release Form (link below). Completed and signed by the parent or guardian. Submitted then to the Safety Director.

Insurance might be available for players and volunteers. Information about insurance and claims forms are available through your Safety Director and online. If you have any questions or problems, you may contact the Safety Director.  Please note that the injured party and/or his or her parents are solely responsible for filing  any claim
